Phantoms are translucent spirits of creatures that died a particularly violent death. A phantom appears much as it did in life, though its form is clearly translucent and incorporeal. Phantoms have no attack form other than causing fear.

Notice Perception DC 15 (to hear the sound of wailing)
hp 6; Trigger proximity (30 ft.); Reset 1 day
Effect When this haunt is triggered, the phantom rushes towards the nearest target, crying and wailing. All creatures in the cone are targeted by a fear spell (save DC 15).
Destruction A banishment or dismissal spell must be cast on the phantom.
